Background
Psychiatric mother and baby units (MBUs) are recommended for severe perinatal mental illness, but effectiveness compared with other forms of acute care remains unknown.
AimsWe hypothesised that women admitted to MBUs would be less likely to be readmitted to acute care in the 12 months following discharge, compared with women admitted to non-MBU acute care (generic psychiatric wards or crisis resolution teams (CRTs)).
MethodQuasi-experimental cohort study of women accessing acute psychiatric care up to 1 year postpartum in 42 healthcare organisations across England and Wales. Primary outcome was readmission within 12 months post-discharge. Propensity scores were used to account for systematic differences between MBU and non-MBU participants. Secondary outcomes included assessment of cost-effectiveness, experience of services, unmet needs, perceived bonding, observed mother–infant interaction quality and safeguarding outcome.
ResultsOf 279 women, 108 (39%) received MBU care, 62 (22%) generic ward care and 109 (39%) CRT care only. The MBU group (n = 105) had similar readmission rates to the non-MBU group (n = 158) (aOR = 0.95, 95% CI 0.86–1.04, P = 0.29; an absolute difference of −5%, 95% CI −14 to 4%). Service satisfaction was significantly higher among women accessing MBUs compared with non-MBUs; no significant differences were observed for any other secondary outcomes.
ConclusionsWe found no significant differences in rates of readmission, but MBU advantage might have been masked by residual confounders; readmission will also depend on quality of care after discharge and type of illness. Future studies should attempt to identify the effective ingredients of specialist perinatal in-patient and community care to improve outcomes.

This work reports the synthesis of undoped and alkali metal doped thermochromic vanadium dioxide thin films by sol-gel spin coating and subsequent low-temperature annealing at 450 °C in N2-H2 atmosphere. The effect of sodium and potassium on the phase transition temperature as well as on the solar modulations were investigated. A dopant concentration of 0.3 at% resulted in a reduction of the critical transition temperature (Tc) from 62 °C to 57 °C and 47 °C for the sodium and potassium doped films, respectively. Moreover, both dopants improved the solar modulations (ΔTsol) of the undoped VO2 films from 3.81 to 9.44 and 5.43 %, respectively.
Making Telecare desirable rather than a last resort

Despite reported benefits of Telecare use for older adults, uptake of Telecare in the United Kingdom remains relatively low. Non-users of Telecare are an under-researched group in the Telecare field. We conducted 22 qualitative individual semi-structured interviews to explore the views and opinions of current non-users of Telecare regarding barriers and facilitators to its use, and explored considerations which may precede their decision to accept, or reject, Telecare. Framework analysis identified a number of themes which influence the outcome and timing of this decision, including peace of mind (for the individual and their family), the strength and composition of an individual's support network, the impact of changing personal and health circumstances, and lack of communication about Telecare (e.g. advertising). A cost–benefit decision process appears to take place for the potential user, whereby the benefit of peace of mind is weighed against perceived ‘costs’ of using Telecare. Telecare is often perceived as a last resort rather than a preventative measure. A number of barriers to Telecare use need to be addressed if individuals are to make fully informed decisions regarding their Telecare use, and to begin using Telecare at a time when it could provide them with optimal benefit. Although the study was set in England, the findings may be relevant for other countries where Telecare is used.

An Early Mesolithic Seasonal Hunting Site in the Kennet Valley, Southern England

A small-scale excavation, undertaken in advance of building works at Faraday Road, Newbury, Berkshire, encountered an apparently intact Early Mesolithic layer containing abundant worked flint directly associated with animal bones. The site lay on the floodplain of the River Kennet in an area already well-known for Mesolithic remains and certainly represents an extension of the site found at nearby Greenham Dairy Farm in 1963. The flint assemblage was dominated by obliquely-blunted microlithic forms accompanied by a restricted range of other items. The animal bones were, unusually, dominated by wild pig with clear evidence of both primary butchery and food waste. Spatial analysis of the bone and flint assemblages indicated discrete activity areas, possibly associated with hearths. Both pollen and molluscan data were recovered which, together with the results of soil micromorphological examination, confirmed an Early Holocene date for the formation of the Mesolithic layer. Radiocarbon dates place the site in the late 10th–early 9th millennium BP. The paper re-examines the nature of known Early Mesolithic activity in this part of the Kennet valley, with particular reference to the specific environmental conditions that seem to have prevailed. It is concluded that the Faraday Road site represents one part of a continuum of Early Mesolithic occupation that stretches along a considerable length of the floodplain, with each focus of activity witnessing repeated, but intermittent, occupation spanning a period of more than a millennium.
